DO I NEED A DOCTOR'S REFERRAL?
Yes, a faxed referral from your primary care provider is required for an initial evaluation. It is also helpful if a diagnosis list and most recent visit note are included. Referrals can be faxed to (360) 282-0715.
WHAT TYPES OF HEALTH INSURANCE DO YOU ACCEPT?
We accept all Medicare insurance policies.
We are unable to accept Kaiser, Well Care or Molina.
WHAT IS YOUR NEW PATIENT PROCEDURE?
Once we receive a referral from your physician and we verify your insurance we will reach out to you to schedule an evaluation.
